Application can be made through online form or alternatively, you can download form applications via the official website Social Welfare Department of Sarawak and Ministry of Welfare, Community Wellbeing, Women, Family and Childhood Development (KWKPK)

The form also can be obtained from the Sarawak State Social Welfare Office (JKMNS), Resident Offices, District Offices and Sub-District Offices throughout the state.

All completed forms can be returned for processing at Sarawak State Social Welfare Office (JKMNS), Resident Offices, District Offices and Sub-District Offices throughout the state or to the Early Childhood Development Division, KWKPK at following addresses:-

Eligibility requirements:

(i) a Malaysian citizen;
(ii) the mother of the Sarawak Identity Card holder with a 'K' status or the Wife of the Sarawak Identity Card holder with a 'K' status;
(iii) Give birth within or outside Sarawak; and
(iv) Registration is made within the period of 22 weeks of pregnancy up to 3 months after birth.
