This application allow the villagers to propose a cemetery plot in their village or to set up Muslim Cemetery committee or register muslim cemetery with approval from Majlis Islam Sarawak.

  2. Copy of Commitee representative MyKad
  3. Surat Wakaf
  4. Surat Pemilikan Tanah
  5. Grave pictures  if any (for existing grave )
